13. 表證 Exterior Pattern
問, 表證.
What is exterior pattern?
脈浮 floating pulse
發熱惡寒, 身體痛而, 脈浮者, 表證也. 浮, 表陽也. 其脈, 按之不足, 擧止有餘. 《素問》云, 寸口脈而盛曰病在外, 寸口脈沈而緊, 病在中. 仲景云, 脈浮者, 病在表可發汗. 又曰表有病者, 脈當浮. 結胸證, 脈浮者, 不可下. 則知脈浮者, 表證也.

Fever, aversion to
cold, body aches, and a floating pulse are all signs of exterior pattern. Floating pulse is a manifestation of exterior pattern, as well as a
manifestation of Yang pattern. When palpating the pulse, it is not apparent
when pressing deeply, but becomes clear when lifting the fingers. In the
"Su Wen," it is said that when the radial artery is strong at the cun
position, it is an external disease, and when the radial artery is sinking and
tight, it is an internal disease. According to Zhongjing, when the pulse is
floating, sweating should be induced for exterior pattern, and when there is a
disease in the exterior, the pulse is floating. For chest congestion syndrome,
precipiating should not be used when the pulse is floating. Therefore, it can
be understood that a floating pulse is a manifestation of exterior pattern.
惡寒 Averson to cold
表證者, 惡寒, 是也. 惡寒者, 表之虛, 此屬太陽, 宜汗之. 然, 傷寒發表, 須當隨病輕重, 而汗之. 故仲景有發汗者, 有和解之者, 兼四時發汗. 亦自不同.
Exterior pattern includes the sign of aversion
to cold in which there is a deficiency on the surface of the body, and it
belongs to the Taiyang stage, so inducing sweating is appropriate. However,
when inducing sweating for Shanghan disease, it must be done according to the
severity of the disease. Therefore, the book written by Zhongjing has different
methods of inducing sweating, harmonizing the body, and inducing sweating in
accordance with the appropriate seasons. These methods vary depending on the
condition being treated.
春發汗法
Method of inducing sweating in spring
春不可大發汗, 以陽氣尙弱, 不可亟奪, 使陰氣勝, 於時, 天寒初解, 榮衛腠理緩, 可用小柴胡湯之類.
In the spring season, excessive sweating
should be avoided as Yang-Qi is still weak and should not be frequently
exhausted, which can lead to an imbalance of Yin and Yang. with Yin-Qi becoming
dominant. At this time, the coldness of the weather is beginning to dissipate,
and the functions of construction and defense, and the the interstices are
gradually relaxing. Therefore, it is appropriate to use formulas such as Xiao Chai Hu Tang (Minor Bupleurum Decoction).
冬發汗法
Method of
inducing sweating in winter
冬不可汗者, 以陽氣伏臧, 不可妄擾. 不問傷寒中風, 以輕藥解利之. 傷寒無汗者, 只與桂枝麻黃各般湯, 傷風有汗, 只與柴胡桂枝湯, 或得少汗而解, 或無汗自解. 病勢甚者, 不拘此.
It
is not appropriate to sweat in winter because it is a time when yang qi
retreats inward and should not be disturbed recklessly. For diseases such as
cold damage and wind strike, only light medicines should be used to resolve the
condition. If there is no sweating in a patient with cold damge, only 桂枝麻黃各半湯 (Cinnamon Twig and Ephedra Half and-Half Decoction, Gui Zhi Ma
Huang Ge Ban Tang) should be administered, and if the patient has sweating due
to damage to wind, only 柴胡桂枝湯 (Bupleurum and Cinnamon
Twig Decoction, Chai Hu Gui Zhi Tang) should be used. In some cases, the
condition can be resolved with little sweating or without sweating. In severe cases, do not be restricted by these points.
