99. When in cold damage that has lasted for five or six days or wind strike, there is alternating aversion to cold and heat effusion; the person suffers from fullness in the chest and rib-side, and taciturnity with no desire for food or drink, heart vexation and frequent retching, or possibly there is vexation in the chest and no retching, or thirst or pain in the abdomen, or a hard glomus under rib-side, or palpitations below the heart with inhibited urination, or absence of thirst with mild generalized heat, or cough; then Minor Bupleurum Decoction (xiao chai hu tang) governs.
99. 傷寒五六日 中風 往來寒熱 胸脅苦滿 默默不欲飲食 心煩喜嘔 或胸中煩而不嘔 或渴 或腹中痛 或脅下痞硬 或心下悸 小便不利 或不渴 身有微熱 或咳者 小柴胡湯主之
When in cold damage that has lasted for five or six days, qi gets to locate
in reverting yin, hence, reverting yin is linked to lesser yang, lesser
yang appears at midterm, hence reverting yin moves qi through depending of the
pivot action of lesser yang. Cold damage and wind strike are as like this, the
sign of alternating aversion to cold and heat effusion is
the indication of the pivot action of lesser yang. The
chest is the place where greater yang governs and rib-side is the place
where lesser yang governs, greater yang fails to come out and lesser yang is
unable to pivot, therefore the person suffers from fullness in the chest and rib-side. The
letter of 默(taciturnity) is composed of 火
(fire) and 黑 (black? Dark?), the fire of hidden brightness is
depressed and fails to stretch, therefore a tacitunn image is observed. Depressed
wood and fire in the center cause stomach network to be disharmonious, as a
result, no desire for food or drink occurs. Alternately hyperactivity of wood and fire gives way to heart vexation, wood likes orderly reaching and tends to ascend,
therefore frequent retching occurs. This is that the disease qi is in the
greater yang and the channel qi is in reverting
yin. ‘Qi appearing in the center (中氣, 中見之氣)’ of reverting yin is lesser
yang that governs pivot. Generally the qi of lesser yang wanders and moves on
the triple burner, altogether eleven depots, they receive decisions from the
gallbladder, therefore seven possible signs is accompanied. If the qi
interferes with only the heart except the stomach, vexation
in the chest and no retching occurs. if interferes with dry qi of yang
brightness, thirst results. If interferes with the qi of greater yin spleen,
pain in in the abdomen occurs. if interfeses with the qi of reverting yin
liver, a hard glomus under rib-side rib-side occurs. if interfere the qi of lesser
yin kidney, palpitations below the heart with inhibited urination occurs. if
there is the force of greater yang coming outward depending on the pivot action
of lesser yang, absence of thirst with mild generalized heat occurs. if
interferes with the qi of greater yin lung, cough occurs.
Generally, back transport points of five viscera channels are in greater
yang, the qi of five viscera comes out from the chest
and controls greater yang. Here, if the qi
of greater yang counterflows to the chest and fails to come out to the
exterior, even though this does not interfere with visceral true (藏真) that has a
form in the interior, but interferes with visceral qi that is formless in the
exterior, therefore various signs are abseved. Without depending
of the force of the pivoting and transportation action, it is hard to turn and
repel the evil that interferes and invades to outward, leading to failure of resolution.
This is treated with Minor Bupleurum Decoction (xiao chai hu tang) which assist
the pivot action.
This line describes that if the qi of greater yang fails to come in and out
through the chest, this will counterflow between the chest and diaphragm and
interfere with visceral qi in the interior, hence it is appropriate to let the evil
come out to the exterior relying on the pivot action of lesser yang.
傷寒五六日 經盡一周 氣值厥陰
藉其中見之少陽而樞轉. 傷寒如此 中風亦如此. 其證往來寒熱 少陽之樞象也. 胸為太陽之部 脅為少陽之部 太陽不得出 少陽不得樞 故為苦滿.「默」字從火從黑 伏明之火鬱而不伸 故其形默默. 木火鬱於中 致胃絡不和 故不欲飲食
木火交亢故為心煩 木喜條達而上升 故喜嘔. 此病氣則在太陽 經氣則值厥陰
厥陰之中見則為主樞之少陽也. 蓋少陽之氣游行三焦 在臟腑之外 十一藏皆取決之 故兼或然七證 或涉於心而不涉於胃 則胸中煩而不嘔 或涉於陽明之燥氣 則渴 或涉於太陰之脾氣
則腹中痛 或涉於厥陰之肝氣 則脅下痞硬 或涉於少陰之腎氣 則心下悸而小便不利 或太陽藉少陽之樞轉 已有向外之勢 則不渴 身有微熱 或咳者 又涉太陰之肺氣矣. 夫五藏之經俞在背 主於太陽 而五藏之氣由胸而出 亦司於太陽. 今太陽之氣逆於胸而不能外去 雖不干動在內有形之藏真 而亦干動在外無形之藏氣 現出各藏之證 非得少陽樞轉之力 不能使干犯之邪向外而解 必與以小柴胡湯助樞以主之.
此一節言太陽之氣不能從胸出入 逆於胸膈之間 內干動於藏氣 當藉少陽之樞轉而外出也.
