,    Generally warm disease first invades the upper burner, and the position of the disease is in greater yin.


    Cold evil in cold damage enters from fur and pores, this arrives in upprer part from lower part. This occurs in the foot greater yin [bladder] channel. The foot greater yin bladder is related with water, cold is a kind of qi fo water, water and cold are same kind and associate with each other, therefore cold damage begins to occur from greater yang bladder channel. From the ancient days so far, medical mans only says that bladder controls the exterior, this is incomprehensive. The lung is connected to the skin and body hair, the lung alos controls the exterior. (thinking about one organ and one bowel of the body, (in other words the lung and bladder, people unintentionally know about and do not study the theory of both lung and bladder controlling the exterior. In respect of great principle of heaven, human, and earth (三才), heaven great exterior of all things and is related with metal. The lung of the body is also related with metal, therefore the lung controls the skin and body hair. Nei Jing (內經) says that the skin corresponds to heaven and heaven engenders water. The Earthly Branches (地支) begins from (Zi), (Hai) becomes heaven gate and in which (Zhen) and (Yuan) meet together, the bladder of the body is the bowl of cold water. therefore both the lung and bladder are related with heaven qi and control the exteriortherefore the treatment of cold damage should follow six channel shift of Zhang Zhong Jing (張仲景).

Warm disease invades from mouth and nose and descends from the upper part, nose frees to the lung, therefore warm disease begins at hand greater yin. Greater yin is related with metal, warmth is the qi of fire, wind is th mother of fire, fire always restrains metal, therefore warm disease begins at hand greater yin. So the treatment for warm disease should be established according to the theory of triple burner of Liu He Jian (劉河間).

And cold is yin evil, -although wind strike is mentioned in Shang Han Lun but thiw wind is also chill wind from west-north side,- is strongest in the nature of contraction, exuberant yin always damages yang, therefore blocks the flow of yang qi of greater yang channel, and this causes headache, generalized heat, and so on. Foot greater yang bladder is related with yang and bowel, cold damage is yin evil, exuberant yin damages the yang of the body. Warmth is yang evil, - although wind damage is also mentioned in this book but this wind is also warm wind from east side- is strongest in the nature of effusing [outflowing], exuberant yang always damage yin, therefore blocks the flow of yin qi of hand greater yin, and this causes cough, spontaneous sweating, thirst, headache, generalized heat, and so on. Hand greater yin is related with yin and organ, warm and heat [evil] are yang evil, exuberant yang damages the yin of the body. Now, the differentiation of yin and yang of warm disease may be cleared in mined.

    Generally sun rises from east, moon rises from west, there is nothing engendered by the qi lesser yang and lesser yin. Therefore all things are called Dong Xi (西, east and west). Man, as the lord of all creation, is supplied with the qi of Dong Xi (西) perfectly, so man corresponds to the qi of heaven, earth, and Dong-Xi (西). Therefore the disease cannot but correspond to the qi of heaven, earth, and Dong-Xi (西). Dong Xi (西, east and west) is the paths of yin and yang. What goes form east to south becomes wood, wind, damp, fire, and heat, damp earth in the middle communicating with fire confirms summerheat. Fire is related with south, what goes from weat to north becomes metal, dryness, water, and cold. Water is related with north. Therfore water and fire are the signs of yin and yang. South and north are extreme of yin and yang. Heaven and earth move this yin and yang and transform and engender all things. Therefore it is said that great favor comes from heaven. If the yin and yang which are moved by Heaven and earth are harmonious, the yin and yang of the body are also harmonious, how can disease be! When there is any deviation in the yin and yang of man, heaven and earth, disease will soon happen. If the deviation is slight, the disease is also slight, if the deviation is deep, the disease is also deep. If the deviation declines to fire, it causes disease with warm character, if the deviation to water, it causes disease with cold charater. This divides disease into water and fire largely, practitioners must know this. Disease related with water should be treated with wamring and heating, disease related with fire should be treated with cooling and clearing. When treating each disease, one just make the deviation to be harmonious. One must not insist one method in clearing, heating, warming, and cooling. Therefore I records that cold disease is originated from water and warm disease is originated from fire.
 
二、凡病温者,始于上焦,在手太阴

    
伤寒由毛窍而入,自下而上。始足太阳。足太阳膀胱属水,寒即水之气,同类相从,故病始于此。古来但言膀胱主表,殆未尽其义。肺者,皮毛之合也,独不主表乎(按人身一脏一腑主表之理,人皆习焉不察。以三才大道言之天为万物之大表,天属金,人之肺亦属金,肺主皮毛,经曰皮应天,天一生水;地支始于子,而亥为天门,乃贞元之会,人之膀胱为寒水之腑;故俱同天气,而俱主表也)!治法必以仲景六经次传为祖法。温病由口鼻而入,自上而下,鼻通于肺,始手太阴。太阴金也,温者火之气,风者火之母,火未有不克金者,故病始于此,必从河间三焦定论。再寒为阴邪,虽《伤寒论》中亦言中风,此风从西北方来,乃发之寒风也,最善收引,阴盛必伤阳,故首郁遇太阳经中之阳气,而为头痛身热等证。太阳阳腑也,伤寒阴邪也,阴盛伤人之阳也。温为阳邪,此论中亦言伤风,此风从东方来,乃解冻之温风也,最善发泄,阳盛必伤阴,故首郁遏太阴经中之阴气,而为咳嗽自汗口渴头痛身热尺热等证。太阴阴脏也,温热阳邪也,阳盛伤人之阴也。阴阳两大法门之辨,可了然于心目间矣。
    
夫大明生于东,月生于西,举凡万物,莫不由此少阳、少阴之气以为生成,故万物皆可名之曰东西。人乃万物之统领也,得东西之气最全,乃与天地东西之气相应。其病也,亦不能不与天地东西之气相应。东西者,阴阳之道路也。由东而往,为木、为风、为湿、为火、为热,湿土居中,与火交而成暑,火也者,南也。由西而往,为金、为燥、为水、为寒,水也者,北也。水火者,阴阳之征兆也;南北者,阴阳之极致也。天地营运此阴阳以化生万物,故曰天之无恩而大恩生。天地营运之阴阳和平,人生之阴阳亦和平,安有所谓病也矣!天地与人之阴阳,一有所偏,即为病也。偏之浅者病浅,偏之深者病深;偏于火者病温、病热,偏于水者病清、病寒,此水火两大法门之辨,医者不可不知。烛其为水之病也,而温之热之;烛其为火之病也,而凉之寒之,各救其偏,以抵于平和而已。非如鉴之空,一尘不染,如衡之平,毫无倚着,不能暗合道妙,岂可各立门户,专主于寒热温凉一家之论而已哉!瑭因辨寒病之原于水,温病之原于火也,而并及之。
