1-1. white vaginal discharge. 白帶下
Generally vaginal discharge belongs to damp pattern. The reason of naiming with ‘dai (帶, belt) is that this occurs due to ‘belt vessel’ failing to retain. Generally belt vessel connects to conception and governor vessels. The diseases of conception and governor vessels influence belt vessel to get disease. Belt vessel has a function of retaining of placenta. When belt vessel is weak, it becomes hard to retain placenta, then placenta will be insecurity. Therefore it is said that when blet vessel is weak, abortion occurs easily and when belt vessel is damaged, fetus is not strong. In the reason of the damage of belt vessel, there is not only falls, wrenching, contusion, but also sexual intercourse, or drinking of wine, and mainia. Although there is no suffering of pain, but there is damage which cannot be seen, qi fails to transform into menstrual water(經水), instead, this changes into vaginal discharge disease. Therefore, in the person who suffers vaginal discharge disease, there is a widow, a Buddhist monk, and woman who is married. A virgin with vaginal discharge disease is few. Moreover, the deficiency of spleen qi, the stagnation of liver qi, invading of damp qi, and pressure of heat qi are added, how can vaginal discharge disease not occur? Therefore white vaginal discharge flows downward as tears or saliva for many months or all year long, and cannot be controlled, in severe case, there is a foul smell. This is called white vaginal discharge. Generally white vaginal discharge is due to exuberant damp with debilitation of fire and liver depression with qi weakness. Spleen earth get damaged, the qi of damp earth falls downward, therefore spleen essence fails to contain and transform into contruction-blood to be menstrual water,instead this changes into something white and slippery. This directly descends from Yin Gate, this has a desire to pause spontaneously, but that is impossible. This should be treated with tonifying the qi of spleen and stomach greatly, and using small dose of herbs which soothe the liver as a assistant. When those herbs which soothe the liver, make wind wood not to be blocked in the earth, then earth qi ascends spontaneously to heaven, when spleen qi is healthy, damp qi disperses, therefore white vaginal discharge releases. Wan Dai Tang (完帶湯) is used to this.

Bai Zhu(one liang, stir bake with soil) Shan Yao(one liang, stir bake) Ren Shen(two qian) Bai Shao Yao(five qian, stir bake) Che Qian Zi(three qian, stir bake with wine) Cang Zhu(three qian)Gan Cao(one qian)Chen Pi(five fen)Hei Jie Su(five fen)Chai Hu(six fen)
Decoct the herbs with water. Two Ji (劑) make this light, four Ji (劑) makes vaginal discharge checked, six Ji (劑) makes white vaginal discharge to recover perfectly. This formula treats the three channels of spleen, stomach, and liver an one time. This tonifies in the middle of dissipating and dissipates in the middle of ascending. When the qi of liver wood opens and ascends, liver blood is not dry, at this condition, how can the liver restrain spleen earth? When the source of spleen earth is supplemented, spleen qi is not damp, how is it difficult to separate and dissipate water qi? Spleen cannot be exuberant without the strength of stomach qi. The supplementing of stomach means the supplementing of spleen.
【outline】 generally white vaginal discharge is related with damp. The basic pathomechanism of white vaginal discharge is liver depression with spleen deficiency and exuberant damp with debilitation of fire. Wan Dai Tang (完帶湯) is used to sooth liver qi which makes spleen qi exuberant, this makes damp qi eliminated, then white vaginal discharge recovers spontaneously.
1-1. 白帶下
夫帶下俱是濕症. 而以帶名者, 因帶脈不能約束, 而有此病, 故以名之. 蓋帶脈通於任, 督, 任, 督病而帶脈始病. 帶脈者, 所以約束胞胎之系也. 帶脈無力, 則難以提系, 必然胎胞不固, 故曰帶弱則胎易墜, 帶傷則胎不牢. 然而帶脈之傷, 非獨跌閃挫氣已也, 或行房而放縱, 或飮酒而顚狂, 雖無疼痛之苦, 而有暗耗之害, 則氣不能化經水, 而反變爲帶病矣. 故病帶著, 惟尼僧, 寡婦, 出嫁之女多有之, 而在室女則少也. 況加以脾氣之虛, 肝氣之鬱, 濕氣之侵, 熱氣之逼, 安得不成帶下之病哉! 故婦人有終年累月下流白物, 如涕如唾, 不能禁止, 甚則臭穢者, 所謂白帶也. 夫白帶乃濕盛而火衰, 肝鬱而氣弱, 則脾土受傷, 濕土之氣下陷, 是以脾精不守, 不能化榮血以爲經水, 反變成白滑之物, 由陰門直下, 欲自禁而不可得也. 治法宜大補脾胃之氣, 稍佐以舒肝之品, 使風木不閉塞於地中, 則地氣自升騰於天上, 脾氣健而濕氣消, 自無白帶之患矣. 方用完帶湯.
完帶湯
白朮(一兩, 土炒) 山藥(一兩, 炒) 人參(二錢) 白芍(五錢,炒) 車前子(三錢,酒炒) 蒼朮(三錢,製)甘草(一錢)陳皮(五分)黑芥穗(五分)柴胡(六分)
水煎服. 二劑輕,四劑止,六劑則白帶全愈。此方脾、胃、肝三經同治之法,寓補於散之中,寄消於升之內,開提肝木之氣,則肝血不燥,何至下克脾土,補益脾土之元,則脾氣不濕,何難分消水氣。至於補脾而兼以補胃者,由里以及表也。脾非胃氣之强,則脾之弱不能旺,是補胃正所以補脾耳。
